By using the Pet Assure card, the holder acknowledges that Pet Assure does not guarantee, nor is it responsible for, the quality, performance, or non-performance of any services or products provided by any provider. All claims regarding products or services provided shall be directed to the veterinary practice or other providers. Pet Assure disclaims any and all warranties, including any implied warranties of merchantability or fitness for a particular purpose, applicable to the program or any products or services provided to subscribers. Pet Assure further disclaims any and all liability for loss due to the action or condition of any participating business.
Participating providers are subject to change without notice and you must confirm with the provider that they are participating prior to your visit. For an updated list, please visit www.petassure.com. Participating providers and other merchants are independent veterinarians and contractors. They are neither agents nor employees of Pet Assure.
Unless you cancel your subscription, your subscription will automatically renew based on the terms of your plan. Each renewal shall constitute a separate and distinct subscription contract for the applicable period. Subscription benefits are limited to the household of the cardholder named on the enclosed card. Benefits are non-transferable, except where applicable.
Your subscription may be cancelled within 45 days of the commencement of the subscription. To cancel, you must return your card and all pet tags to Pet Assure's return address via certified mail. Or cut your card in two and email a picture to firstname.lastname@example.org. If proof of return cannot be established, or if your card is not returned, your fee will not be refunded. Processing and shipping fees are non-refundable. Membership cards will be mailed within 1 - 10 business days.
